Consultant Pharmacy Services
Saint Mary's Health Care Consultant Pharmacists are all
members of the American Society of Consultant Pharmacists.
This helps ensure they are on the cutting edge of consultant
pharmacy practice
- The Consultant Pharmacist will assist with facility QA
Committee in resident assessment and preparing the care
plan
- The Pharmacist will ensure the medication use process
meets OBRA mandates and other regulations
- Will review the medication regimen of each resident
monthly to identify any potential or actual drug related
problems, including contraindications, unrelated indications,
improper drug selection, sub-therapeutic dosage, failure
to receive drugs, over dosage, adverse drug reactions,
drug interactions, and drug use without indication
- Participate, where appropriate, in the drug therapy
decision making process
- Recommend appropriate drug and non-drug therapies for
use by the resident based upon the disease or condition
under treatment, other medical problems, and physiological
changes related to the aging disease process
- Assist physicians, other prescribers and residents in
establishing therapeutic and/or functional outcomes for
each medication prescribed
- Identify, evaluate and make recommendations to ameliorate
the effects of noncompliance in residents
- Participate in resident assessment and care planning
- Initiate and or modify drug therapy, and monitor vital
signs and laboratory assessments in cooperation with
the attending physician, in accordance with the facility
policies and procedures
- The Consultant Pharmacist will attend one scheduled
committee meeting per month and at least the quarterly
QA meeting at your facility
- Computer assisted durg use review & reports
